As a participant in our program, you or your daughter will be able to access a private section of our website with a variety of program-specific resources. Please create a user name and password below.

Note: If you’re applying to the Junior Elite program, this access will be tied to the parent or guardian’s email address and for their use as well.

Note: If you’re applying to the Senior Elite and/or Cotillion programs, this access will be for the participant and will be tied to the participant’s email address.

• I will follow the rules made to protect others and myself and to help make sure program activities are safe, fun and successful.
• I will treat other people, property, equipment and myself with respect.
• I will keep all conversations at EWOE events confidential and acknowledge that
• I will not behave in such a manner that monopolizes the time of the facilitator or jeopardizes the EWOE experience for all of the participants.
• I will not bully, harass or abuse my fellow EWOE participants.
• I will listen carefully to all instructions and appropriately respond to all directions given.
• I will be respectful of the EWOE facilitators, volunteers, and directors.

Failure to follow our Code of Conduct may result in dismissal from the program at EWOE’s discretion.

EWOE reserves the right, based on the behavior involved, to terminate a participants’ involvement in EWOE or its programs.

FINANCIAL AGREEMENT

Participant's Full Name

1. A non-refundable Registration Fee of $35 must accompany this completed Contract (Payment will be accepted after the signing of this enrollment contract)
2. The participant and the participant's parent/guardian agree to comply with and be subject to the EWOE rules and policies as set forth in the Parent-Participant Handbook, as amended from time to time.
3. In all events, the participation fee payment must be paid on or before the first workshop or event, or the participant's place will not be reserved.
